The Trial Court has issued a revised G.L. c. 209A Order Form. Attached is the Trial Court’s announcement, which includes instructions for ordering the updated 209A carbon copy packets. Also attached are PDF-fillable copies of the revised 209A Order Form and Return of Service for departments that prefer to continue using the fillable application materials on mass.gov.
Please share this with your shift supervisors and anyone else in your department who handles emergency orders after court hours.
